Diagnostic Services: Licenses & Accreditation

All testing locations at UW Medicine Pathology are subject to the Clinical Laboratory Improvement Amendments of 1988 (CLIA-88).   Washington State's clinical laboratory licensure program has been judged by the Federal Health and Human Services Centers for Medicare and Medicaid Services (CMS) as equivalent to CLIA and was granted an exemption from the federal Clinical Laboratory Improvement Amendments of 1988 (CLIA).  Our CLIA number is maintained by the Washington State Department of Health as part of the Medical Test Site licensure program.

Our laboratories are located at the University of Washington Medical Center, UW Health Sciences Center and Harborview Medical Center.  All locations have individual licenses and accreditations. Please refer to the list below for each locations' licenses, image of certification and testing services.  UW Medicine has never experienced a limitation, suspension, probation or voluntary relinquishing of a license, certification or accreditation.  The certifications are linked below.

Record, Specimen, and Clinical Material Retention

Hospital pathology laboratories must retain records, specimens and clinical material in a way that meets or exceeds the more stringent of CAP accreditation standards or CLIA regulations as published by these entities.
